What is periodontal disease?

“Perio” means around and “dontal” means related to teeth. Periodontal disease which affect the structures around the teeth, includes gums, periodontal ligament and surrounding (alveolar) bone.

Types of periodontal disease:

1. Gingivitis
2. Periodontitis

4. Disease related periodontitis
5. Necrotizing periodontal disease

Types of periodontal disease:

Gingivitis:
As a result of plaque and tartar build up on teeth, bacteria cause the gums inflammation which called “gingivitis”. Usually present as a red swollen gums which can bleed easily on provocation.

It is a reversible form of gums disease usually cured by regular brushing, flossing and scaling by a dentist.

Periodontitis:
when gingivitis left untreated it will progress in a more severe form of gums disease called periodontitis. It’s a chronic inflammation of gums and surrounding structure of the teeth which may leads to tooth loss. Characterized by:

Red, swollen gums which will bleed easily.
Bad breath.
Purulent (pus) discharge from gums.
Periodontal abscess.
Tooth migration.
Gums recession.
Deep pocket (> 3mm) formation due to separation of gums tissues from the tooth.
Loose (mobile) teeth.

Different types of periodontitis

Chronic periodontitis: it’s a most common type of periodontitis which usually start in young age but unnoticeable due to absence of pain.
Aggressive periodontitis: generally affects the systemically healthy young adult around puberty. Rapid rate of attachment loss usually start around the 1st molar and incisors. Radiographic finding shows the arc shaped bone loss from distal surface of 2nd premolar to mesial surface of 2nd molar.
Disease related periodontitis: periodontitis present as a manifestation of systemic disease like, diabetes, AIDS, Down syndrome, several disorder of white blood cells.
Necrotizing periodontal disease: it is an unusual form of acute gums infection. The onset and progression of disease is very fast, present as a painful, ulcerated bleeding gums. Sometimes due to ignorance it may spreads to the facial area and cause severe damage. Most commonly seen in a patient of AIDS and malnutrition.

Risk factors for gums disease:

Poor oral hygiene
Smoking
Pregnancy or hormonal imbalance in women.
Medical condition like diabetes, AIDS, malnutrition.
Genetic susceptibility
Medication: medicine phenytoin use to treat seizures and another medicine cyclosporine use to suppress the immune system may cause the xerostomia (dryness of mouth) leads to more accumulation of plaque followed by gums inflammation. Nifedipin and calcium channel blockers used in case of blood pressure and heart disease may cause abnormal overgrowth of gums tissues.

Root planning

instrumentation to remove the microorganism from the root surface or lying free in the pocket, tarter, contaminated cementum and dentin in order to return a healthy non inflamed state of gums.

Surgical treatment

Flap surgery
if patient did not get the desire treatment outcome with regular cleaning and root planning surgery might be useful. A periodontist will reflect the flap (gums tissues from the root and bone surface), remove the tarter, and thoroughly clean the root surfaces and pocket then sutured back around the tooth.

Bone and tissue grafts
sometimes in addition to flap surgery you might need a placement of bone or tissue grafts. Grafting is also known as a GTR (guided tissue regeneration) help in new growth of bone and gums tissues destroyed by periodontitis.
Medication: along with the periodontal treatment, below drugs are also prescribed to control the bacterial growth.
